MIDDLEFIELD - David A. Sly, 50, died Thursday morning, Dec. 6, 2012, at UH-Geauga Medical Center, Claridon.
He was born June 1, 1962, in Claridon, the son of Jesse and Oris Welchman Sly.
Survivors include a daughter, Jamie Sly; a stepdaughter, Angie Davis; and siblings, Linda Williams, Wanda Sly, and Butch and Brian Sly.

A celebration of life will be at 11 a.m. Tuesday at Best Funeral Home in Middlefield, where family and friends will be received from 5 to 8 p.m. Monday.
